This information will be used by RMH staff involved in outpatient scheduling and care coordination. It may be shared with clinical teams involved in your treatment.
If you do not provide the requested information, we may not be able to process your appointment change.
Your information will be handled in accordance with the Health Records Act 2001 (Vic) and the Privacy and Data Protection Act 2014 (Vic).
